Transfer Coordinator - Transfer Center
This position will be dedicated to facilitating appropriate (requiring a higher level of care than what is available at the patient's current location) patient transfers into Alta Bates Summit Medical Center (Alta Bates or Summit campus as appropriate) from referring physicians and facilities (hospitals, clinics, etc.). Person filling this position will ensure an appropriate level of care bed within ABSMC and will facilitate a timely mode of transportation. Responsible for meeting the needs and expectations of our referring physicians/facilities, as well as our accepting physicians, by supporting and managing the patient transport process efficiently and effectively.
This position has no direct reports and reports to the Manager of Program Development who is under the administrative directive of the ABSMC Vice President of Medical Affairs.
Requirements:
Education: Valid California Bachelor Degree in Nursing or related field is desirable. Registered Nurse license is required.
Experience: Five or more years clinical nurse experience required. Recent acute care experience and/or position such as in Emergency room/triage, ICU, nurse manger, nursing supervisor, or case manager preferred. Demonstrated excellence in customer service skills. Experience with data collection is desirable
Knowledge: Familiar with applicable state and federal standards and regulations, such as EMTALA, related to acute and urgent/emergent patient transports. Knowledge of State, Federal, and Professional industry standards pertaining to health care practice (e.g. Title 22 and JCAHO). Working knowledge of procedure for the admission and inter-facility transfer of patients is preferred. Working knowledge of all state Medicare and Medi-Cal guidelines preferred
Special Skills/Equipment: Excellent customer service skills, demonstrating an ability to place significant focus on communication and teamwork. Excellent interpersonal and communication skills, both oral and written. Ability to deal objectively with a wide variety of situations. Ability to manage multiple priorities in stressful situations. Basic computer skills and the knowledge required to support a nursing/transfer center information system. Ability to work independently and be a self-starter
Work collaboratively on projects with personnel from a variety of service lines. Able to understand complex diagnosis and treatment information and use deductive reasoning.
Physical Requirements: Intermittent sitting and walking-not anticipated to require lifting or excessive (in excess of ½ hour) standing or walking.
