Position Summary:
The Senior Service Coordinator, HUD-VASH Case Management will facilitate the successful achievement of program participants’ housing and community integration goals as a member of an interdisciplinary team.
Among your responsibilities, you will:
* Implement effective case management/care coordination systems that address complex problems facing homeless veterans transitioning to and maintaining permanent housing in the community, and primarily, assist same with resolving barriers and finding/maintaining stable housing. * Have an understanding of supportive housing and apartment finding, mental illness, substance abuse disorders, public systems and issues affecting veterans inclusive of the HUD-VASH program and services provided by the Veterans’ Affairs (VA) system and other relevant systems of care including clinical supports. Requirements
Successful candidates will have:
* New York State Licensed Social Worker (LMSW) or Master’s level clinician (MA) Required * Demonstrated experience in the vocational rehabilitation / supportive housing field * Substantive experience working with veterans * Computer literate, MS/Word & Excel.
